Auswahl von Systemansätzen für komplexe, multiperspektivische Managementsituationen im Gesundheitswesen/ vorgelegt von Marc Andreas Bürgi
Die Schweiz ist eines der OECD-Länder, in welchem die Gesundheitskosten schneller wachsen als das BIP. Aufgrund des staatlichen und wirtschaftlichen Veränderungsdrucks stehen Entscheidungsträger aktuell vor der Aufgabe, Veränderungsprojekte mit hoher technischer sowie sozialer Komplexität zu bewältigen. Solche komplexe, multiperspektivische Managementsituationen können mittels Systems Thinking moderiert werden. In der vorliegenden Arbeit wird basierend auf dem Paradigma des Critical Systems Thinking untersucht, wie bestehende Systemansätze ausgewählt werden sollen, welche Ordnungsrahmen dazu bereits bestehen und wie diese in einer Entscheidungshilfe abgebildet werden können. Das Ziel der vorliegenden Arbeit ist dabei die Konstruktion einer integrierten Entscheidungshilfe für die Auswahl von Systemansätzen. Dazu werden Ansätze der UK Systems Thinking Tradition nach Kriterien qualitativer Forschung mittels der Design Science Research Methodik sowie anhand von Fallstudien untersucht. Basierend auf Literaturrecherchen werden Merkmale bestehender Ordnungsrahmen kombiniert und anschliessend 20 Fallstudien aus dem Gesundheitswesen damit analysiert. Das Resultat ist eine Entscheidungshilfe, welche die Auswahl von Systemansätzen anhand bestimmter Merkmale einer Problemsituation unterstützt. Aufgrund fehlender Kooperationspartner konnte dieses Artefakt nicht praktisch evaluiert werden. Dennoch wird mit der integrierten Entscheidungshilfe eine wissenschaftlich begründete Hypothese zur Auswahl von Systemansätzen im Gesundheitswesen bereitgestellt. Zusätzliche Erkenntnisse werden präsentiert, welche die Paradigmen des Hard Systems Thinking, Soft Systems Thinking sowie Critical Systems Thinking integrieren. Der Fokus wird dazu von einem Methodenwahl-Modus des Critical Systems Thinking neu auf einen Reflexions-Modus gelegt. Die persönliche Reflexion und Entwicklung von Anwendern stehen dadurch schliesslich als normative Ziel. - Switzerland is one of the OECD countries in which the growth of health service costs exceeds that of the GDP. Due to growing governmental and economical pressure, decision makers are facing the challenge to execute change projects which exhibit increased technical and social complexity. Such problematic management situations can be supported by means of applying Systems Thinking. Based on the Critical Systems Thinking paradigm this thesis investigates how existing systems approaches should be chosen, which frameworks support such a decision and how these can be used in order to build a decision-making aid. The final goal of the thesis is the design of such an integrated decision-making aid for the health care system. To this end, systems approaches in the UK Systems Thinking tradition are investigated on the basis of qualitative research criteria, the Design Science Research methodology and case studies. Supported by a first and a second literature research, criteria are extracted from existing frameworks and thereafter assessed with data gathered from 20 case studies concerning the health care system. Hence an integrated decision-making aid results which supports the choice of systems approaches depending on specific criteria of the problematic situation. Due to the lack of corporate partners for research collaborations the artefact could not be evaluated in practice. Nonetheless, it represents a sound scientific hypothesis on how to choose systems approaches for health service systems. Additional research findings are presented which suggest the integration of the Hard Systems Thinking, Soft Systems Thinking and Critical Systems Thinking paradigm. The focus of a method-choice-mode of Critical Systems Thinking is therefore shifted towards a reflection-mode.
