"??rztenetzwerke" als Reformmodell f??r den ambulanten Sektor? Diese Frage steht im Mit-telpunkt der vorliegenden Arbeit. Ausgangspunkt der Untersuchung ist die Feststel-lung, da?? die institutionelle Fragmentierung des ambulanten Sektors, die aus einer organisato-rischen, rechtlichen und finanziellen Trennung zwischen haus??rztlicher und fach??rztlicher Versorgung resultiert, zu erheblichen Qualit??tseinbu??en und Kostensteigerungen in der Gesundheits-versorgung f??hrt. Als Antwort auf diese institutionellen Schwachstellen werden in der derzeitigen Reformdebatte zwei konkurrierende Modelle diskutiert: ??rztenetzwerke und integrierte Versorgungsformen nach Vorbild der amerikanischen Health Maintenance Organizations (HMO's). Vor diesem Hintergrund sind zwei Fragestellungen Gegenstand der folgenden Untersu-chung. Zum einen wird n??her analysiert, ob ??rztenetzwerke aus ??konomischer Perspektive geeignet sind, die Fragmentierung der Versorgungsstrukturen im ambulanten Sektor zu beheben. Wenn ??rztenetzwerke als Alternative zur derzeitigen Organisation der ambulanten Versorgung zu empfehlen sind, so wird zum anderen die Frage des effizienten Netzwerkaufbaus und -arrangements beantwortet. Da keine umfassenden theoretischen oder empirischen Studien ??ber Gesundheitsnetzwerke vorliegen, ist das Vorgehen in dieser Arbeit weitestgehend explorativ. Inwiefern Netzwerke zur Verringerung der institutionellen Schwachstellen im ambulanten Sektor beitragen k??nnen, ist abh??ngig von den konkreten Ursachen der Leistungsver-schlechterungen. Daher erfolgt zun??chst eine eingehende Darstellung des ambulanten Sektors und eine Ursachenanalyse. Hierauf aufbauend wird aus theoretischer Perspektive n??her untersucht, inwiefern Netzwerke zu einer Verringerung dieser Leistungsverschlechterungen beitragen k??nnen. Zu diesem Zweck wird die netzwerkliche Organisationsform aus zwei Gr??nden der marktlichen sowie der hierarchischen bzw. inte-grierten Organisation gegen??bergestellt. Auf der einen Seite stellen Netzwerke aus institutio-nen??konomischer Sicht eine Synthese der beiden "Idealtypen" Markt und Hierarchie bzw. Integration dar, so da?? ??ber eine netzwerkliche Organisation theoretisch jene Elemente der beiden Systeme miteinander kombiniert werden k??nnen, die f??r die Organisation der Gesund-heitsversorgung als besonders effizient angesehen werden k??nnen. Auf der anderen Seite erfolgt ein Vergleich der Effizienz von Netzwerken und integrierten Versor-gungsformen, da diese in der aktuellen Reformdiskussion als Alternative zum derzeitigen System pr??feriert werden. Bei der Gegen??berstellung der beiden "Idealtypen" stehen vor allem f??nf Strukturmerkmale im Vordergrund: die r??umliche Organisation der Leistungserstellung, die Koordinations- und Kontrollmechanismen, die Verteilung der Eigentums--rechte und die Kosten der Vertragsausgestaltung zwischen Versicherern und Anbietern. Hieran anschlie??end wird dargestellt, da?? ??ber eine netz-werkliche Organisation die Vorteile der beiden idealtypischen Formen kombiniert werden k??nnen. Unter der Annahme, da?? das Netzwerkarrangement, das sich aus verschiedenen Netzwerk-regelungen und der Netzwerkstruktur zusammensetzt, Einflu?? auf den Anreiz zur Einhaltung der Netzwerkverpflichtungen und die Wahrscheinlichkeit opportunistischen Verhaltens aus??ben k??nnen, steht im weiteren Verlauf der Arbeit die Frage des effizienten Arrangements im Mittelpunkt. Im Gegensatz zur ersten Fragestellung wird diese sowohl theoretisch als auch empirisch untersucht werden. Unter Ber??cksichtigung ver-schiedener soziologischer sowie institutionen??konomischer Theorieans??tze werden Hypo-thesen ??ber einen optimalen Netzwerk--aufbau abgeleitet. Diese theoretischen Ableitungen werden anhand von Gesundheitsnetzwerken in der Schweiz n??her ??berpr??ft.
